The State of Maine requires all students from Kindergarten and on to have 5 DTP, 4 Polio 2 MMR and 1 Varicella Immunization or documentation of chicken pox disease from a physician.

Philosophical or religious exemptions from immunizations need to be filed annually with the school nurse (Maine School Immunization Law 20-A MRSA as 6532-6538).  You can request a form from the school nurse or it is located on the 'Health Forms'  page on this site.

If your child is not up to date with their immunizations, you must inform the school nurse of their physician's 'catch-up' schedule.
