COVID-19 Information for 23/24 School Year

To report a positive COVID-19 results please fill out the Positive COVID-19 Report Form

The district is continuing to follow the US CDC guidelines which are based on an individual's symptoms. At this time only positive individuals need to remain home. People who have been exposed to COIVD-19 may continue normal activity including school attendance though masking for a full 10 days after exposure is highly encouraged. The CDC recommends closely monitoring for symptoms and testing at least 5 full days after your last exposure even if you do not develop symptoms. If at any time symptoms do develop, stay home and test.

Your child’s school nurse can help with decisions regarding returning to school. For any questions or concerns regarding COVID-19 please contact your school nurse.

Health Forms

Each year Emergency Health Information Forms need to be completed for all students. Students needing daily prescribed medications during the school day and students with chronic medical conditions also need Medication Administration Forms and/or Health Care Plans completed every school year, even if no changes have occured since the last school year. In some cases we may need a Release of Information Form completed to allow for communication between your school nurse and your student's health care provider regarding their health condition.
